1. Name
2. Delivery Address 

3. Delivery mode (normal or registered mail: you need to pay if delivery by registered mail.)

4. Free delivery to the following MRT stations for self collection (Strongly suggested!!!)

- Clementi MRT station 
- Outram Park MRT station (after 7:30 weekday)

- City Hall MRT station (after 7:30 weekday)

By the way, we can discuss delivery time and place if it is convenient for both of us.
